The 14 GPs that have been ordered to improve by a watchdog can be revealed – and this is what inspectors had to say about them.

Independent inspectors from the Care Quality Commission (CQC) regularly visit GPs to find out whether the service is safe, effective, caring, responsive and well-led.

There are four ratings given to health and social care services – these are Outstanding, Good, Requires Improvement and Inadequate.

Below are the 14 GPs that have been rated as 'Requires Improvement', along with what inspectors had to say about them.

The inspectors said: "The registered person failed to assess, monitor and improve the quality and safety of the services provided in the carrying on of the regulated activity."

They also found "the records of staff vaccinations were not up to date".

The inspectors wrote: "The provider did not have effective arrangements in place for the monitoring and actioning medicines safety alerts.

"This was in breach of Regulation 12(1) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014."

They added: "Consent to minor surgery was not audited and the patients consent had not been comprehensively recorded."

"There was no evidence of an induction program and staff appraisals had not all been completed."

The inspectors said: "Performance for diabetes and hypertension related indicators for 2017/ 2018 was below local and national averages.

"The practice had taken action and unverified data showed that improvements to performance in both these indicators had taken place.

"Published results showed the childhood immunisation uptake rates for the vaccines given were lower than the target percentage of 95% or above in three out of the four indicators."
